Case study: Turning an eTMF health check into a clear path to inspection readiness

Nov 6, 2026 | Clinical Operations, Quality & Business Excellence

Overview

An international vaccine-development organisation engaged Pharmalys to conduct an independent health assessment of the electronic Trial Master File for an ongoing clinical study. The assessment covered approximately 800 documents maintained in an eTMF platform. Its purpose was to evaluate completeness, document quality, organisation and inspection readiness against applicable Good Clinical Practice expectations, study procedures and the TMF Reference Model.

Background

Ahead of a planned audit and study close-out, the sponsor needed a clear and objective understanding of the condition of its eTMF. The review needed to identify documentation gaps, filing and metadata inconsistencies, potential compliance risks and areas requiring immediate remediation. The sponsor also needed practical recommendations that could be assigned, monitored and verified.

Pharmalys’ approach

Pharmalys performed a detailed assessment covering:

  • TMF structure, artifacts and expected content
  • Essential documents completeness
  • Missing, duplicate and misfiled records
  • Documents legibility, signatures and accuracy
  • Metadata completeness and consistency
  • Document naming convention consistency
  • Filing timeliness • Version control and audit-trail compliance
  • Potential inspection-readiness risks

Observations were documented in a controlled quality-review tracker and consolidated into an executive close-out summary.

Key observations

The assessment identified several recurring themes:

  • Repeated misfiling, particularly involving trial-team training records and committee documents
  • Incorrect or inconsistent version dates, signature dates, labels and naming conventions
  • Missing, incomplete, uncontrolled, editable or bundled records
  • Outdated GCP evidence and incomplete trial-team documentation
  • Personally identifiable information (PII) retained in CVs and requiring redaction

These findings demonstrated that eTMF readiness depends not only on whether documents are present, but also on whether they are complete, correctly filed, controlled, traceable and appropriately protected.

From findings to action

Pharmalys translated the observations into a prioritised remediation approach. Recommended actions included:

  • Assigning an accountable owner and target date of resolution to every observation
  • Prioritising PII redaction, missing essential records, uncontrolled documents and date or signature discrepancies
  • Correcting filing locations, document names, metadata and document formats
  • Replacing bundled ZIP packages with individually filed records
  • Performing second-level quality-control reviews and retaining evidence of completed remediation

Value delivered

The assessment provided the sponsor with:

  • An independent, evidence-based view of overall eTMF health
  • A structured, traceable record of all observations
  • A risk-based remediation sequence focused on the highest-priority issues
  • Clear accountability, ownership and target dates for each action
  • A defined quality-control step to verify completed remediation
  • A stronger foundation for audit readiness and efficient study close-out

The assessment established a clear baseline and a practical remediation roadmap for audit readiness and study close-out. It enabled the sponsor to focus first on the issues most likely to compromise document reliability, privacy, traceability and inspection response.

Conclusion

eTMF inspection readiness is not a one-time filing exercise. It requires continuous attention to completeness, quality, and evidence of oversight.

A focused eTMF health assessment can make recurring weaknesses visible before an audit, translate them into accountable actions and establish a practical route toward sustained inspection readiness.
